Informed Consent and Waiver
I, {name}, hereby acknowledge that I have voluntarily chosen to participate in fitness training sessions provided by Honor Bound FIT. I understand that participation in physical fitness activities involves certain inherent risks, including but not limited to muscle soreness, sprains, strains, fractures, cardiovascular complications, and in extremely rare cases: serious injury or death.
I acknowledge that the trainers and staff at Honor Bound FIT are not medical professionals and cannot provide medical advice or diagnosis. I understand that it is my responsibility to consult with a physician before beginning any exercise program, especially if I have any pre-existing medical conditions or concerns.
I understand that the trainers at Honor Bound FIT will make every effort to provide safe and effective training sessions, but they cannot guarantee results or prevent all injuries. I agree to disclose any relevant medical history, injuries, or concerns to my trainer prior to beginning any fitness program.
I acknowledge that I am participating in fitness training sessions of my own free will and assume all risks associated with such participation. I release Honor Bound FIT, its trainers, employees, and affiliates from any and all liability for injuries, damages, or losses that may occur as a result of my participation in fitness training sessions, including but not limited to negligence on the part of Honor Bound FIT or its staff.
I understand that I have the right to ask questions and seek clarification about any aspect of the fitness training program offered by Honor Bound FIT. I agree to follow the instructions of my trainer and to inform them immediately if I experience any pain, discomfort, or other symptoms during training sessions.
I understand that this consent form will remain in effect for the duration of my participation in fitness training sessions at Honor Bound FIT unless I choose to revoke it in writing.
I have read and understand the terms of this informed consent and waiver, and I voluntarily agree to its provisions.
